This is used for seriously ill cases, regarded as imminent death (less than 12 months to live), for active or deferred members. Please issue the relevant Medical Consent Form and complete the relevant Part 1 Form.

The member needs to provide the employer evidence of their serious ill health in the form of a report from a Specialist/GP/Specialist Care Provider such as Macmillan for example, which needs to include their diagnosis and prognosis.
