This process is employer led - having exhausted your own sickness monitoring procedures and the member is unlikely to return to work, on the advice of your medical practitioners/occupational health you may wish to look at ill health retirement. After a formal discussion with the member, if going down this route, complete the IHR Task Analysis and send a copy of this along with the Ill health your Questions answered fact sheet and Medical Consent form to the member for them to complete and return to you.
Whilst waiting for the completed forms you can complete IHR Part 1 and if you do not have a panel doctor arrangement set up – engage the services of a panel doctor to take on the case.
