ill Health Assessment for Deferred Members

This process is member led- an ex-employee needs to apply to their former employer for the release their deferred benefits on the grounds of ill health via letter or email, stating their current health situation and providing any supporting documents they may have. You need to know when the member left employment to be able to select and complete the correct paperwork from the forms below. Please complete the DB1 Forms for members who left on or after 01/04/2008 and the DB2 Forms for members who left on or before 31/03/2008. Please complete the relevant Task Analysis and send a copy of this along with the relevant Guidance sheet and Medical Consent Form to the member for them to complete and return to you.

Whilst waiting for the completed forms you can complete the relevant DB Part 1 Form and if you do not have a panel doctor arrangement set up – engage the services of a panel doctor to take on the case.
